One of my all time favorite intros is Xasthur's Entrance into Nothingness on Telepathic with the Deceased. It has such a cold and eerie atmosphere you can almost feel the cold crawling on your skin. Really draws you into the album as a good intro should:

I couldn't find the intro for United Aryan Evil on youtbe but the Fullmoon demo intro deserves a mention anyways! As well the prolog for Wolfnacht's Toten für W.O.T.A.N...

Its not technically a full intro track, but Truppensturm - Obliteration of All Beings is a fucking wicked way to begin an album.
