A record that I was supposed to press another label picked up, the Scumguts S/T LP, the label put the outro and the last two songs twice on side B, clearly didn't listen to the test pressings.

His label got shut down and he disappeared, band was really crushed by it.

epitaph - seeming salvation cd on konqueror records - awful , slowed down remix/remater.sold it away after one listen.
torchure - essence on vic records - just a bad remaster , like many of their releases.

The BoB repress of the Hate Forest record Battlefields had some power metal band on the b-side.

I bought a Tragedy record a couple years ago that had some power pop band's album on it. It wasn't bad.

I'm sure I have more but this is the one that pisses me off the most so I remember it the worst.

2016 Kvlt pressing of DDTM on CD has Tireheb as the last track instead of the first.

I think at least the Black Legions Metal vinyl edition by Mysticum Prod got fucked up by additional noise which doesn't belong on the record. It was later reissued by Drakkar/Niessedrion.

Agatus "Dawn of Martyrdom piclp by Iron Pegasus at least came with a insert notifying that the first song has problems, hehe.

Blood - Impulse To Destroy vinyl reissue with tracks that are supposed to blend together having a noticeable split, as if they were taken from mp3s or something. Also, it's got horrible pixelated artwork and overall shit sound.

The Accüsed - Martha Splatterhead's Maddest Stories Ever Told. The tracklist and tracks are all fucked up. As I put in the MA additional notes, "the tracks listed as "Deception" and "Molly's Xmas '72" are actually the song "Smothered Her Thrust" split in two. Molly's Xmas '72" is the intro to "I'd Love to Change the World", which is included in the same track even though it's listed as a separate one. The track listed as "Losing Your Mind" is actually "Deception". The track listed as "Smothered Her Thrust" is actually "Losing Your Mind"."
And in my particular copy, the A side's final groove that's supposed to spiral into the center of the record so the turntable switches off (I don't know the technical term for this) is just a circular groove, which means the end just loops over and over and you have to pull out the needle by hand. I don't know if this is present in all the copies or just a fault on my own copy, but due to the nautre of the A side's outro, it actually sounds like it could be done on purpose because the loop sounds perfect and you could sit there for a minute or two without realizing it's fucked up.

Actually it's uncredited covers of a really obscure Nidhogg tape from '94, by an unknown band.

From Nidhogg himself:

"iLDjARN has confirmed to me that there’s been a mix-up in regards to the source-material for the split. The songs are my original compositions in their entirety, though uncredited. Old tapes have been floating around . There is no quarrell between me and iLDjARN regarding this, it’s a matter between him and OSMOSE.

The preliminary synth-basses here represent parts of what was later to be recorded as guitar and bass. The songs are 6 of intentionally 12, in random order, with original lyrics drawn from and tracking Voluspå, the ancient Norse tale of Ragnarok.

In the order they were recorded, with original titles, on my ca. ’94 sketch-tape:

1. Balders skjebne
2. Syner
3. Fimbulsvintr
4. Garm gjøy
5. Det siste slage"

To be honest the original version its the only one i know. What do you mean by fucked up? The "snareless" fast beats? The pink logo?
That record is pretty fucking perfect to me. Never heard the remastered or remixed version...
